We collect basic information, such as but not limited to your name, age, sex, nationality, religion, date of birth, occupation, address and contact information, major health complaint/s, current/previous medical history, vital signs, laboratory results and ancillary services (if any), and other information that could help us provide you with a better service. We will as well collect relevant contact or personal information of your next of kin or guardians.
WHY WE COLLECT THEM. The information gathered is used for your medical treatment. These data help us understand your current health status and provide you the necessary healthcare needed. We maintain your record for efficient coordination between healthcare members who are involved in managing your health concerns. We may also utilize your personal data for:
HOW WE USE, STORE, AND RETAIN THEM. Your data is stored in medical records office under strict security controls and policies and is only accessible by authorized staffs and security personnel. We dispose the physical and electronic data after Fifteen years (15) years from the date of collection unless required by law to retain them for a longer period. Besides from DOH, your HMO, and agencies with whom you are affiliated with, we do NOT transfer or share your personal data with other persons or organizations, unless with your consent, and required or permitted by law.
